Tooth decay is more prevalent than diseases like cancer, heart disease or influenza. It doesn't have to be—brushing with fluoride toothpaste, flossing, less dietary sugar and regular dental cleanings can lower the risk of this harmful disease.
Hygiene, diet and dental care work because they interrupt the disease process at various points. Daily hygiene and regular dental cleanings remove dental plaque where oral bacteria flourish. Reducing sugar eliminates one of bacteria's feeding sources. With less bacteria, there's less oral acid to erode enamel.
But as good as these methods work, we can now take the fight against tooth decay a step further. We can formulate a prevention strategy tailored to an individual patient that addresses risk factors for decay unique to them.
Poor saliva flow. One of the more important functions of this bodily fluid is to neutralize mouth acid produced by bacteria and released from food during eating. Saliva helps restore the mouth's ideal pH balance needed for optimum oral health. But if you have poor saliva flow, often because of medications, your mouth could be more acidic and thus more prone to decay.
Biofilm imbalance. The inside of your mouth is coated with an ultrathin biofilm made up of proteins, biochemicals and microorganisms. Normally, both beneficial and harmful bacteria reside together with the “good” bacteria having the edge. If the mouth becomes more acidic long-term, however, even the beneficial bacteria adapt and become more like their harmful counterparts.
Genetic factors. Researchers estimate that 40 to 50 hereditary genes can impact cavity development. Some of these genes could impact tooth formation or saliva gland anatomy, while others drive behaviors like a higher craving for sugar. A family history of tooth decay, especially when regular hygiene habits or diet don't seem to be a factor, could be an indicator that genes are influencing a person's dental health.
To determine if these or other factors could be driving a patient's higher risk for tooth decay, many dentists are now gathering more information about medications, family history or lifestyle habits. Using that information, they can introduce other measures for each patient that will lower their risk for tooth decay even more.

You've heard it. Your parents heard it—maybe even your grandparents too. Dentists have been alerting people for more than half a century that high sugar consumption contributes to tooth decay.
That message hasn't changed because the facts behind it are the same in the 2020s as they were in the 1950s: The bacteria that cause tooth decay feast on sugar and other leftover carbohydrates in the mouth. This causes them to multiply and increase their production of acid, which softens and erodes tooth enamel.
What has changed though, especially over the last couple of decades, is a growing understanding of how sugar consumption may affect the rest of the body. Just like the evidence of sugar's relationship to tooth decay, current scientific studies are now showing there are strong links between sugar and diseases like diabetes, heart disease and liver disease.
What's startling about what researchers are finding is that cases of these diseases are growing, Especially in younger people. This is a parallel trend to our skyrocketing increases in per capita sugar consumption: the average American now eats the equivalent of 19.5 teaspoons of added sugar every day. Health experts generally agree we should consume no more than 6 teaspoons a day, and children 4.
This is vastly more than we consumed a generation ago. One reason is because processed food manufacturers have increased sugar in their products, hiding under technical, unfamiliar names in ingredient lists. But it's still sugar, and an estimated 74% of processed foods contain some form of it.
But the real surge in sugar has come from our increasing consumption of sodas, as well as energy and sports beverages. These beverages are high in sugar—you can meet your daily allowance with just one 12-oz can of soda. These beverages are now the leading source of sugar in our diets, and, according to experts, a highly dangerous way to consume it.
In effect, dentists of old were on to something: too much sugar is bad for your teeth. It now turns out that it may be bad for your overall health too. Strictly limiting it in your family's diet could help lower your risk of tooth decay and dangerous diseases like diabetes.
From the moment your child's first tooth appears, usually between six and nine months, you need to be concerned about Early Childhood Caries (ECC). This particular form of tooth decay can have a devastating effect on primary (baby) teeth and lead to their premature demise. Losing one before its time could adversely affect how the future permanent tooth comes in.
You can help prevent ECC with daily brushing and cleaning, regular dental visits (beginning around their first birthday) and limiting the sugar they eat. Here are 3 more things to consider for boosting your prevention efforts.
Breastfeeding. Pediatricians generally recommend breastfeeding if possible for a baby's overall health, including dental development. And although breast milk contains fermentable carbohydrates that boost bacterial growth, it no more promotes tooth decay than similar foods and beverages. That said, though, once the child begins to eat and drink other foods and beverages, the combination of sugars in them and breast milk could increase the bacteria that causes ECC. This is another good reason to wean the child from breast milk as they begin to eat more solid foods.
Bottles and pacifiers. It's quite common for parents and caregivers to soothe a fussing or crying baby with a bottle filled with formula, milk or juice for sipping, or even a pacifier dipped in jam, sugar or some form of sweetener. But these practices can create an environment that promotes high acid production from bacteria feeding on the sugars. Instead, avoid giving them a “prop-up” bottle filled with liquids containing sugar and try to limit bottle use to mealtimes. And provide them pacifiers without sugary additives if you use them.
Medicines. Children with chronic illnesses or other needs often take medication containing sugar or with antihistamines that reduce the flow of acid-neutralizing saliva. If the medications can't be altered, then it's extra important for you to practice diligent, daily hygiene to reduce the effect of higher mouth acid.

False—decayed primary teeth can lead to potentially serious consequences later in life.
Although “baby” teeth last only a few years, they’re essential to future dental health because they act as placeholders and guides for the incoming permanent teeth. If they’re lost prematurely due to decay, other teeth may drift into the empty space intended for the emerging permanent tooth. Because of this, inadequate space will crowd the out of proper alignment.
And because they have thinner enamel than permanent teeth, primary teeth are more susceptible to decay. Once decay sets in, it can spread rapidly in a matter of months.
Fortunately, we may be able to prevent this from happening to your child’s primary teeth with a few simple guidelines. It all begins with understanding the underlying causes of tooth decay.
Tooth decay begins with bacteria: As a result of their digestion, these microorganisms secrete acid that at high levels can erode tooth enamel. The higher the population of bacteria in the mouth, the higher the acidity and potential threat to the teeth.
The first objective then in preventing decay is to remove dental plaque, the thin film of bacteria and food particles on tooth surfaces, through daily brushing and flossing. And because bacteria feed on sugar as a primary food source, you should reduce your child’s sugar consumption by restricting it to only meal times and not sending your child to bed with a bottle filled with a sugary liquid (including formula or breast milk).
To help boost your child’s protection, we can also apply sealants and fluoride to teeth to help protect and strengthen their enamel from acid attack. Because we’ll also monitor for signs of decay, it’s important to begin regular dental visits beginning around age one. If we do detect decay, we can then treat it and make every effort to preserve your child’s primary teeth until they’ve completed their normal life cycle.
By taking these steps, we can help make sure your child’s early teeth go the distance. Their current and future dental health will certainly benefit.

Other than the common cold, tooth decay is the most prevalent disease in the world. And while a cavity or two may seem like a minor matter, tooth decay’s full destructive potential is anything but trivial. Without proper prevention and treatment, tooth decay can cause pain, tooth loss and, in rare cases, even death.
This common disease begins with bacteria in the mouth. Though these microscopic organisms’ presence is completely normal and at times beneficial, certain strains cause problems: they consume left over carbohydrates in the mouth like sugar and produce acid as a byproduct. The higher the levels of bacteria the higher the amount of acid, which disrupts the mouth’s normal neutral pH.
This is a problem because acid is the primary enemy of enamel, the teeth’s hard protective outer shell. Acid causes enamel to lose its mineral content (de-mineralization), eventually producing cavities. Saliva neutralizes acid that arises normally after we eat, but if the levels are too high for too long this process can be overwhelmed. The longer the enamel is exposed to acid, the more it softens and dissolves.
While tooth decay is a global epidemic, dental advances of the last century have made it highly preventable. The foundation for prevention is fluoride in toothpaste and effective oral hygiene — daily brushing and flossing to removing plaque, a thin film of food remnant on teeth that’s a feeding ground for bacteria, along with regular dental visits for more thorough cleaning and examination. This regular regimen should begin in infancy when teeth first appear in the mouth. For children especially, further prevention measures in the form of sealants or topical fluoride applications performed in the dentist office can provide added protection for those at higher risk.
You can also help your preventive measures by limiting sugar or other carbohydrates in your family’s diet, and eating more fresh vegetables, fruit and dairy products, especially as snacks. Doing so reduces food sources for bacteria, which will lower their multiplication and subsequently the amount of acid produced.
In this day and age, tooth decay isn’t a given. Keeping it at bay, though, requires a personal commitment to effective hygiene, lifestyle choices and regular dental care. Doing these things will help ensure you and your family’s teeth remain free from this all too common disease.
